Contest Entry Form:

1).  Use White typing paper
   8 1/2" x 11" max. size.
2).  On the back of the artwork Child's First Name (lightly in pencil)
3).  Please do not fold your child's entry

Criteria for Entry
- Winning Entries  Based on:

1).  Originality/Creativity.
2).  Does design illustration reflect theme.
3). Was color scheme or lack of color appropriate.
4.  All entries - please use white paper. (Essays/poems 75- 100 words).
